Ectropion and entropion are eyelid position problems, not cosmetic concerns. Ectropion means the eyelid turns outward. Entropion means the eyelid turns inward. Both can irritate the eye surface and may need medical or surgical treatment when symptoms are persistent.

These conditions most often affect the lower eyelid, though the underlying causes can vary.

Ectropion symptoms and risks

With ectropion, the lower lid may sag away from the eye. Patients may notice tearing, redness, burning, crusting, mucus, light sensitivity, or the feeling that the eye is exposed. Because the lid is not resting normally against the eye, tears may not drain or spread properly.

Causes can include age-related laxity, facial nerve weakness, scarring, trauma, prior surgery, or skin disease.

Entropion symptoms and risks

With entropion, the eyelid rolls inward so lashes or skin rub the eye. This can cause foreign body sensation, tearing, redness, pain, blurred vision, or corneal scratches. Persistent rubbing can damage the cornea, so evaluation should not be delayed when symptoms are significant.

Entropion can be related to age-related tissue changes, scarring, inflammation, or spasm.

What evaluation looks for

The exam checks eyelid tone, direction of eyelid rotation, corneal health, tear film, facial nerve function, scarring, previous surgery, and whether lashes are touching the eye. Treatment may include lubrication, taping, temporary measures, or surgery to restore eyelid position. The exact operation depends on the cause.

When to seek care

Prompt evaluation is important for eye pain, worsening redness, light sensitivity, decreased vision, discharge, or a sensation that lashes are scratching the eye. These symptoms may indicate corneal irritation or injury.

Why lubrication may not be enough

Lubricating drops and ointment can reduce irritation, but they do not always correct the mechanical problem. If the eyelid is turned outward, tears may not spread or drain normally. If the eyelid is turned inward, lashes can continue to rub the cornea. Persistent mechanical irritation can damage the eye surface even when drops provide temporary relief.

Symptoms that deserve faster evaluation

Eye pain, light sensitivity, reduced vision, worsening redness, discharge, or a scratchy sensation that does not improve can suggest corneal involvement. Entropion with lashes rubbing the eye should not be ignored because corneal scratches can progress.
